Dear Dr. Roach:A recent study published inNature Medicineon the efficacy and safety of Viagra for the treatment of erectile dysfunction (ED).
It found that men who took sildenafil had an improvement in erectile function in one-half of the studies. For these men, a total of eight trials comparing the drug with placebo showed an improvement in ED.
“This finding highlights the importance of assessing patients’ sexual experiences in relation to their health status and health resources,” says David A. Heffernan, MD, professor of urology at Boston University School of Medicine. “ED is a common condition and treatment with sildenafil is essential. It should not be overlooked, however, that many of these patients do not respond well to oral sildenafil.”
He added that the current “tendency to prescribe Viagra to these men makes it a convenient treatment option for ED,” which is why Viagra should be considered first-line treatment in most cases. A large number of men with ED are prescribed ED medications, including sildenafil, tadalafil and vardenafil, which are used to treat erectile dysfunction (ED). These medications work in a similar way as Viagra, but they have the same active ingredient.
While Viagra and other erectile dysfunction medications are available over-the-counter, most drugs have the potential to interact with each other. This can lead to unwanted side effects, such as headaches, flushing, indigestion and nasal congestion. If a person takes Viagra regularly, they may experience more severe side effects such as chest pain, difficulty breathing or swelling in their legs and feet.
Dr. Roach advises that patients be aware of possible interactions with certain medications, such as nitrates and alpha-blockers, and inform their health care providers if they experience any side effects.
Roachhas received grants from Merck, Bristol-Myers Squibb and Novartis and was part of the American Urological Association’s PDE5 Study Group.
In the study, of the 8,000 men who took sildenafil to treat erectile dysfunction, of whom 1,847 (96%) had improved sexual function. In the comparison with placebo, the sildenafil group improved ED by 10%, whereas the placebo group improved ED by 15%.
The study’s authors state that this finding should be verified in larger trials, as “many men who did not respond to the drug did so due to potential side effects.”
Heffernan adds that “if a patient was able to maintain an erection and have intercourse for the rest of their lives, it may be a sign of a more serious condition.”
He says the current research is not conclusive, and does not recommend against treatment as long as the patient has been able to achieve and maintain an erection and has had satisfactory sexual intercourse.
“If a patient cannot achieve and maintain an erection, he should consult their doctor.”
Roach added that men should also be aware of the potential side effects of sildenafil, including headache, indigestion and nasal congestion. If these side effects occur, it may be necessary to discontinue treatment.
